Mechanism of Action
Paracetamol's precise mechanism of action is not fully understood, but it is believed to primarily inhibit cyclooxygenase (COX) enzymes in the central nervous system rather than at peripheral inflammation sites. This central COX inhibition reduces prostaglandin E2 (PGE2) synthesis in the hypothalamus, lowering the body's temperature set point to produce its antipyretic effect. Its analgesic action may also involve activation of descending serotonergic pain inhibition pathways and interaction with the endocannabinoid system via its metabolite AM404. Unlike NSAIDs, Paracetamol has minimal anti-inflammatory activity and does not inhibit platelet aggregation, making it the preferred first-line analgesic for patients who cannot tolerate NSAIDs — including those with peptic ulcers, asthma, or bleeding disorders.
