Description

Quinine Sulfate is one of the oldest and most established antimalarial drugs, derived from the bark of the cinchona tree. It remains a critical treatment option for chloroquine-resistant P. falciparum malaria and is listed on the WHO Model List of Essential Medicines.

Quinine is particularly important in the management of severe and complicated malaria where ACTs may not be available or suitable. It is also used as a second-line treatment and for malaria in pregnancy (second and third trimesters).

Mechanism of Action

Quinine is a cinchona alkaloid that accumulates in the acidic food vacuole of the Plasmodium parasite. Inside the vacuole, the parasite digests host haemoglobin, releasing toxic free heme as a byproduct. The parasite normally detoxifies heme by polymerising it into insoluble hemozoin (malaria pigment). Quinine inhibits this heme polymerisation process, causing a buildup of toxic free heme that damages parasite membranes and leads to parasite death. Quinine also intercalates into parasite DNA and may interfere with protein synthesis. Unlike artemisinins, which act rapidly but are cleared quickly, Quinine has a longer duration of action (half-life 10–12 hours) and remains effective against chloroquine-resistant P. falciparum strains, making it a critical second-line and rescue therapy.
